Helping people ‘live their best lives’ with health, social care

A program that connects patients with a nurse navigator to help them find the appropriate hospital-based and/or community resources is among numerous community health resources available to area residents.

SCOPE (Seamless Care Optimizing the Patient Experience), a program offered through Oak Valley Health (formerly Markham Stouffville Hospital), was among the resources featured at the Carefirst Community Healthcare event.

Hosted by the Carefirst Seniors & Community Services Association at the Unionville YMCA on September 17, the event brought together 35 community organizations that provide health care-related support services, community support services, wellness and community outreach programs, and a youth volunteer program.

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency that was established in 1976. Its team of multi-disciplinary professionals deliver timely, accessible and patient- and family-centred care in collaboration with its community partners. It’s located in a ‘campus of care’ in Richmond Hill.

Its mission is to empower clients to live their best lives as they age by providing integrated health and social care, such as an adult day program, assisted living services, bereavement support, chronic disease management, an elder abuse hotline, geriatric assessment and intervention network, and home care services.
