The province is introducing Ontario Health Teams, a new model of care that brings together health care providers to work as one team.
“Ontario is delivering on its commitment to end hallway health care and build a connected and sustainable health care system centred around the needs of patients,” MPP Paul Calandra said at the recent announcement of the new Eastern York Region North Durham Ontario Health Team (EYRND OHT).
The EYRND OHT is one of the first 24 teams in the province to implement the new model. The EYRND OHT will create a local health care system that provides coordinated care for patients, reduces wait times, and leads to better health outcomes for patients.
Goals of the new system include making every point of contact safe, humane, and person-centred, embedded in trusting relationships between clients and providers and among providers. Year one priority populations are clients with mental health challenges and addictions, people living with dementia, and caregivers of people living with dementia.
Through an OHT, patients will experience easier transitions from one provider to another, including, for example, between hospitals and home care providers, with one patient story, one patient record and one care plan.
“We will continue to build on the long-standing relationships rooted in innovation and collaboration with our community service partners and local primary care physicians,” said Jo-anne Marr, president and CEO of Markham Stouffville Hospital. “Spanning across two geographic regions, our partners are uniquely positioned and are committed to working with caregivers and people with lived experience to ensure the right connections are made so they can move through the system and access the care they need.”
The EYRND OHT will implement some of their proposed programs and services in 2020 and will communicate with patients and families about the changes and improvements they can expect in their community.
